Case Management
We offer specialty case management services for clients with chronic illnesses. We utilize a collaborative process that assess, assist in planning, implements, coordinates, monitors, links, and evaluates the options and services required to meet the client's health and human service needs.
340 B Pharmacy Services
LCIS helps provide 340B Pharmacy services to clients who meet the eligibility requirements.
Financial Benefits & Budget Counseling
We help clients to understand budget management and refer them to public and private benefit systems. We use the essential community support services of area providers to ensure client needs are met. There are some eligibility requirements for this service.
Ryan White Part B Services
The Ryan White HIV/AIDS Program (RWHAP) Part B allows LCIS to provide support services for clients we serve. There are some eligibility requirements.
HIV/STD Prevention & Education
We provide education to prevent and control sexually transmitted diseases (STDs). We support and make referrals to free testing/treatment sites in our service area.
Medication Assistance
LCIS provides access to free and low-cost medication through pharmaceutical company programs for low income, uninsured patients who cannot afford them. We also provide HIV Medication Assistance Program (HMAP) for clients who meet the eligibility requirements.
Housing Support
LCIS provides Housing Information Services (HIS) to clients who may be at risk of homelessness and to help them gain or maintain access to medical care.
Spiritual Counseling
LCIS provides a faith based spiritual counseling service which provides a safe and nurturing space for clients to explore their inner selves, beliefs, values, and desires to develop a deeper understanding of themselves and to cultivate a stronger connection to their spirituality.
Resource Referrals
LCIS provides referrals to community resources which are beneficial to those who may be in need of outside services.
Life Coaching
A life coach is someone who counsels and encourages clients through personal or career challenges. At LCIS, we have certified life coaching staff to help clients reach their highest potential.
Adherence Counseling
LCIS provides adherence counseling to those clients who are at high risk. Treatment adherence includes starting HIV treatment, keeping all medical appointments, and taking HIV medicines every day and exactly as prescribed. For people with HIV, treatment adherence is key to staying healthy.
Community Outreach Services
LCIS provides additional outreach services that are beneficial to the community and its continued growth.
